Applicants seeking admission to Five Towns College in Long Island, New York must complete the following items:

At this time, a Personal Statement of at least 150 but no more than 300 words is required. This essay can be uploaded and submitted to [email protected] via this link.
All High School and College Transcripts, sent directly from the academic institutions attended. The mailing address is Five Towns College, Admissions Office, 305 N. Service Road, Dix Hills, New York 11746-5871. Please click here if you would like Five Towns College Office of Admissions to submit a Request for Official High School Transcript on your behalf. You can submit an unofficial transcript temporarily but please follow up with the official transcript as soon as possible.
One Letter of Recommendation is required at this time. Please submit a Letter of Recommendation from your guidance counselor, teacher, member of the community or your employer to support your application, not a family member. Student-Athletes should submit a letter of recommendation from their Coach. This letter should be scanned, mailed, emailed or faxed directly from this person to [email protected] or (631) 656-3199.
At this time, applicants for the Music and Theatre programs are permitted to submit online pre-recorded auditions or can request auditions in person directly from the Chairs for Theatre and/or Music. These applicants may be admitted after submitting both the online application and transcripts contingent upon a successful audition.
